Public Health Solutions (formerly Medical and Health Research Association of New York City, Inc.), on behalf of the New York City Department of Health and Mental Hygiene (NYCDOHMH) with authorization from the New York City Council, releases this Request For Applications (RFA) to eligible providers for the purpose of funding programs aiming to decrease HIV/AIDS mortality and morbidity rates in communities of color in New York City. Grant awards are expected to range from $65,000 to $75,000 for Category 1: Access to and Maintenance in Care; and a maximum of $25,000 for Category 2: Condom Distribution. Funds awarded under this RFA must be expended by June 30, 2009.

Public Health Solutions (formerly Medical and Health Research Association of New York City) is releasing an RFP on behalf of the New York City Department of Health and Mental Hygiene (NYCDOHMH), which, through its HIV/AIDS Care, Treatment and Housing Program (CTHP), is making funds available from its Ryan White (Part A and Minority AIDS Initiative-“MAI”) federal grants for the provision of medical and support services and interventions including the following: a) care coordination, (b) health education and promotion, (c) medication adherence, (d) outpatient “bridge” medical care, (e) support and coaching and (f) assistance with social services and benefits to PLWHA in New York City in need of such services. The effective and integrated delivery of these services will ensure that PLWHA are linked to and maintained in care and that they receive the support and social services they need to maintain continuous engagement in HIV primary care and have a “medical home,” to be described in the RFP.
